By completing and signing this form, you grant us permission to publicly share your business information.
Please note: A one-time fee of $20 is required to be listed on our church website. This fee helps cover administrative and maintenance costs associated with managing the directory.
1. Voluntary Participation – I am submitting my business information voluntarily and understand that inclusion in the church directory or promotional efforts is not guaranteed and may be edited for clarity or consistency.
2. Accuracy of Information – I affirm that the information provided is accurate and current to the best of my knowledge.
3. Permission to Share – I grant Mt. Calvary Church the right to:
- List my business on its website.
- Share business information on its social media pages.
- Include my business in printed directories, flyers, or cards distributed by the church.
4. One-Time Fee – I understand that a one-time fee of $20 is required to be listed on the church website.
5. No Endorsement Implied – I understand that while Mt. Calvary Church may promote my business, it does not imply endorsement or assume any liability related to the services or products offered.
6. Right to Revoke – I may revoke this consent at any time by providing written notice to the church. I understand that Mt. Calvary Church will make reasonable efforts to cease further distribution but cannot recall materials already printed or shared.
Any information you provide for your business will be final. Additional fees may apply for updates (sale promos, upcoming events, etc.)
